Refer a member to enhanced care managment

 Enhanced Care Management is designed to help members navigate multiple needs without having to manage everything on their own.

Our Enhanced Care Managers can help members:

• Coordinate medical, behavioral health, dental, and other health-related care
• Connect with housing and shelter resources
• Locate food and nutrition assistance
• Identify transportation resources
• Connect with utility assistance and other community programs
• Navigate benefits, applications, referrals, and paperwork
• Coordinate communication between providers and community organizations
• Address barriers that may make it difficult to access or maintain care
• Develop goals and a personalized care plan
• Follow up on referrals and services to help keep care moving forward

Every member's needs are different. Our team starts by listening, identifying priorities, and helping the member determine the best place to begin.

Doulas Partner provides Enhanced Care Management services to eligible Medi-Cal members who may benefit from additional support coordinating their health care, social services, and community resources.

We serve members throughout our service areas including Shasta County, Siskiyou County, Trinity County, Tehama County including individuals living in rural and remote communities where accessing services can be especially challenging.

Our team works with members who have complex medical, behavioral health, social, or day-to-day needs and would benefit from having a dedicated care manager help coordinate their care and connect them with resources.

Referrals may come from health care providers, community organizations, social service agencies, behavioral health providers, hospitals, clinics, county programs, and other organizations working with individuals who may benefit from Enhanced Care Management.

Potential referral partners may include:

• Primary care and specialty providers
• Hospitals and emergency departments
• Behavioral health and substance use providers
• Social workers and case managers
• Community-based organizations
• Housing and homeless service providers
• County and social service agencies
• Schools and family-serving organizations
• Other professionals supporting Medi-Cal members
